CMS and most accreditation bodies recommend calibrating patient scales at least annually. High-use scales in bariatric units or where medication dosing is particularly weight-sensitive may warrant more frequent calibration. Scales should also be re-calibrated after any repair, after a significant drop or impact, and whenever readings appear inconsistent.

Each calibrated scale receives a written calibration certificate noting the device identifier, as-found reading, adjustment made (if any), as-left reading, tolerance pass/fail, calibration date, next due date, and technician signature.
