What Every Service Report Should Include

  1. Device-level records: asset ID, device type, serial number and location for each unit serviced
  2. What was done: inspection, calibration, parts replaced and tests performed
  3. Measured results where they apply (calibration points, oxygen purity, electrical safety readings)
  4. Failures and follow-up: anything removed from service, and the plan to repair and re-test it
  5. Test equipment used and proof its calibration was current
  6. Technician documentation: name plus training and experience records
  7. Signatures and dates from the technician and your facility

Insurance and Technician Standards

Technicians in the Medical Equipment Repair Network are required to carry at least $1M in general liability insurance and provide a certificate of insurance when they join. Ask your technician for a copy of their certificate before the visit if your facility requires one on file.

Why It Matters at Survey Time

For skilled nursing facilities, surveyors often pick a specific scale, lift or bed and ask for its maintenance record. For PT clinics, state boards, accreditors and insurers may ask the same about modality equipment. A device-level report answers that in seconds. See our CMS survey equipment maintenance checklist and maintenance log template.
