Regulatory Requirements

CMS Conditions of Participation — Skilled nursing facilities must maintain patient handling equipment in safe working order. 42 CFR §483.90(b) requires that facilities maintain all equipment to protect resident health and safety. Patient lift failures that cause resident injuries are consistently cited as deficiencies during surveys.

OSHA Ergonomics — While OSHA does not have a specific patient lift standard, the General Duty Clause (Section 5(a)(1)) requires employers to provide workplaces free from recognized hazards. Malfunctioning patient lifts are a recognized cause of worker injury in healthcare, and OSHA can cite facilities where lifts are not properly maintained.

State regulations — Many states have enacted safe patient handling laws that specifically require regular inspection and maintenance of patient lift equipment. California, Texas, New York, New Jersey, Washington, and other states have statutory requirements in this area.

ASHE/ANSI standards — The American Society for Healthcare Engineering has published guidelines for patient handling equipment, and ANSI/ASSP A10.42 covers requirements for aerial and elevated work platforms (applicable to some ceiling lift systems).

What Lift Inspections Cover

A complete patient lift inspection by a qualified biomedical technician should cover:

Structural inspection — Frame integrity, welds, hardware, and the condition of weight-bearing components. Cracks, bends, corrosion, or deformities in structural members are automatic failure conditions.

Load testing — The lift is tested at its rated capacity (and often at 125% of rated capacity) using calibrated test weights. A lift that cannot safely hold its rated load must be taken out of service immediately.

Drive and brake system — For powered floor lifts and ceiling lift motors, the drive mechanism, speed control, and emergency stop functions are tested. For manual lifts, the pump and release valve are inspected and tested.

Battery and charger system — Battery capacity, charging function, and battery age are assessed. Battery failure during a transfer is a serious safety incident.

Sling attachment points and spreader bar — Carabiner hooks, snap hooks, and spreader bar integrity are critical. Worn or deformed attachment hardware must be replaced before the lift returns to service.

Ceiling track system (for ceiling lifts) — Track mounting hardware, end stops, corner units, and traverse carriage are inspected. Track installations must remain secure in the ceiling structure.

  • When batteries are replaced

Slings should be inspected before every use by the staff member performing the transfer. Worn, frayed, or damaged slings should be removed from service immediately and not used pending replacement or repair. Slings have finite service lives and should be replaced on a schedule regardless of apparent condition.

The majority of patient lift incidents involve sling failure or improper sling use rather than lift mechanical failure. Key sling safety considerations include:

  • Use only manufacturer-approved slings for each specific lift model — cross-brand sling/lift combinations may be unsafe even if they appear to fit
  • Inspect slings for tears, fraying, worn stitching, and hardware deformation before each use
  • Follow manufacturer guidelines for sling laundering — some materials degrade with improper washing
  • Document sling inspections as part of your facility's equipment maintenance program
  • Retire slings on the manufacturer's recommended service life schedule, not just on apparent condition
